If you're a new patient with SMILE Dental, please contact our office at 239-319-2440 so we can set you up with your Online Patient Portal prior to your first visit. The online portal allows you to fill out your paperwork prior to your visit, at your own convenience and with no printing needed.

Your first visit will be a great opportunity to build the foundation of a long-lasting dental relationship. You'll have ample time during your first appointment to get to know your new dentist, Dr. Andisco. Save time on the day of your appointment by filling out your new patient information online.
